What to bring to your first appointment:

In addition to your insurance card, you should bring your driver's license or some other form of identification to your first appointment, as well as the following information:

• List of Medications taken (bring bottles)
• List of Herbal/Holistic supplements taken (bring bottles)
• Existing medical conditions
• List of surgeries
• List of physicians seen
• Family history of diseases
